Johnson Memorial Health has launched a new remote patient monitoring (RPM) and chronic care management (CCM) program in partnership with Prevounce Health. The program is a major step forward in how the hospital supports patients with chronic conditions.
Through the initiative, patients receive cellular-enabled devices that automatically share vital health data with their care team. This real-time connection allows clinicians to detect changes earlier, follow up more quickly, and help patients manage conditions like hypertension, diabetes, and heart failure from the comfort of home.
Integrating technology with personalized care, the teams at JMH are able to meet patients where they are and intervene before small issues become larger concerns.
