Recognition makes the CIN the first accredited clinically integrated network in Indiana and one of only 12 accredited CINs nationwide
The Suburban Care Collaborative Clinically Integrated Network (CIN) has earned URAC Clinically Integrated Network Accreditation, a prestigious designation recognizing excellence in care coordination, quality improvement, patient safety, and population health management.
URAC is an independent national accreditation organization that advances healthcare quality through rigorous standards focused on clinical practice, consumer protections, performance measurement, operational infrastructure, and risk management. By achieving this accreditation, the Suburban Care Collaborative CIN has demonstrated a meaningful commitment to delivering high-quality, patient-centered care while improving clinical outcomes and reducing inefficiencies across the healthcare continuum.
Founded in 2024, the Suburban Care Collaborative CIN is a physician-led collaboration of Suburban Health Organization (SHO) partner hospitals working together to advance value-based care. Through shared clinical leadership, evidence-based care pathways, data analytics, and collaborative best-practice development, participating organizations are strengthening care quality, enhancing patient experiences, reducing avoidable costs, and supporting clinicians in delivering exceptional care.
With this achievement, the Suburban Care Collaborative CIN becomes the first accredited CIN in Indiana and one of only 12 URAC-accredited clinically integrated networks in the United States.
The network currently includes:
- Decatur County Memorial Hospital
- Hancock Health
- Hendricks Regional Health
- Henry Community Health
- Johnson Memorial Health
- Riverview Health
- Schneck Medical Center
- Witham Health Services
“Achieving URAC accreditation is a significant milestone for the Suburban Care Collaborative and a testament to the dedication of our physician leaders, hospitals, and care teams,” said Craig Wilson, MD, President and CEO of Suburban Health Organization and Executive Director of the Suburban Care Collaborative CIN. “This recognition validates our commitment to working collaboratively across independent health systems to improve quality, enhance patient outcomes, and advance value-based care. We are proud to be the first accredited CIN in Indiana and among a select group nationwide recognized for excellence in clinical integration.”
The Suburban Care Collaborative was created to serve as a performance engine for participating organizations and the communities they serve. By bringing independent healthcare providers together around common quality goals and data-driven decision-making, the CIN enables local health systems to remain strong, innovative, and responsive to the evolving needs of patients.
“Clinically Integrated Networks are the future of healthcare as they promote the best in provider-to-provider collaboration to improve patient care,” said Shawn Griffin, MD, President and CEO of URAC. “URAC is proud to recognize the Suburban Care Collaborative CIN for its achievement of Clinically Integrated Network accreditation. This accreditation reflects excellence in the areas of health information technology, care coordination and population health.”
