Accountable Care Organization

The Accountable Care Vision: create a healthier community, improve healthcare quality, and lower healthcare costs. Seven Suburban Health Organization Hospitals (Hancock Health, Hendricks Regional Health, Henry Community Health, Johnson Memorial Health, Logansport Memorial Hospital, Margaret Mary Health and Witham Health Services) are partnering with Caravan Health to participate in the Medicare Shared Savings Program (MSSP). The Suburban Health MSSP ACO’s combined savings over the first six years has been approximately $21 million while maintaining a quality score of 95%.

Suburban Health Organization also participates in other payer Medicare Advantage ACOs. Twelve of the Suburban Health Organization Hospitals have partnered with American Health Network/OptumCare in a United Medicare Advantage ACO. Across the SHO ACOs (MSSP and Medicare Advantage) the key population health initiatives include ensuring the best quality of care, effectively managing chronic care populations, managing the cost of care, and facilitating care coordination across the continuum of care.
