SHO Wins 2026 National Cooperative of Health Networks Association (NCHN) Outstanding Health Network Award

As the nation’s population ages, the rising prevalence of chronic diseases is driving up healthcare costs and placing significant strain on an already overburdened delivery system. At the same time, many patients face barriers to accessing consistent care and struggle to manage their chronic conditions without frequent doctor’s visits. Suburban Health Organization (SHO) is addressing these challenges through innovative, coordinated remote care programs such as remote patient monitoring (RPM) and chronic care management (CCM).

Guided by SHO’s Chief Transformation Officer Mike Gizzi, these programs help member hospitals improve access to care, strengthen patient engagement, and deliver more proactive, efficient support to patients with chronic conditions across Indiana. SHO’s RPM & CCM programs are designed around a simple idea: patients should be able to effectively manage their chronic health conditions from home.

Through cellular-connected medical devices that transmit patient vitals, care teams remotely monitor blood pressure, weight, blood oxygen, and blood glucose readings. SHO member hospitals use a platform and devices from Prevounce Health to manage these programs. The platform uses artificial intelligence and other parameters set by clinicians to analyze patient data in real time. The technology surfaces important changes in a patient’s health and empowers clinicians to respond quickly when issues arise.

Meanwhile, care teams regularly call patients to discuss diet, exercise, and other factors impacting their health. In all, these programs help avoid costly hospitalizations, as well as improve patient outcomes and engagement. A key strength of SHO’s model is coordination. SHO helps align implementation across the network and supports member hospitals through shared planning, proven workflows, and program development guidance. This approach allows members to launch and expand remote care programs more efficiently, as well as to learn from each other’s successes.

SHO’s commitment to innovation and collaboration continues to expand across its service area. SHO members Major Health Partners, Witham Health Services, and Meridian Health Services are beginning to design their own remote care programs with support from SHO and Prevounce. This growth reflects strong network support and demonstrates how SHO’s leadership is helping member organizations improve care access, quality, and efficiency. While many individuals contribute to this work, the vision and coordination provided by Mike Gizzi have been essential in guiding member hospitals through program development.

SHO’s RPM and CCM programs are improving health outcomes, saving money, and creating a more proactive system of care for thousands of patients.
