Amy White, RN

In her role as Clinical Integration Consultant, Amy coordinates clinical transformation efforts across SHO hospital practices in order to maximize performance in population health contracts. She works to ensure that primary care practices incorporate common work flows and care standards and strive to meet shared quality key performance indicators. To do so, Amy works with office managers and local medical directors / primary care champions to develop, maintain, and implement plans to achieve quality improvement goals.

Amy gleans best practices from individual sites to share with the broader SHO community. She works with the external and internal constituents for SHO’s clinical Affinity Groups such as the Tertiary Affiliate Council, Clinical Excellence Council, Primary Care Council, and CMO Roundtable along with the Indiana Hospital Association.

Other responsibilities include:

• Managing strategic and departmental projects
• Coordinating shared services and quality improvement activities
• Facilitating ongoing communications between SHO members
• Providing leadership for the Clinical Excellence Council, CMO Roundtable, CNO Council, and Primary Care Council
