SHO Hospitals Participating in Indianapolis Coalition for Patient Safety Initiatives

The Indianapolis Coalition for Patient Safety (ICPS) is working on several collaborative initiatives to address patient safety in central Indiana hospitals.

SHO’s participation in these initiatives is being led by the Physician Roundtable, with each of the following hospitals participating in a work group along with other ICPS member hospitals:

Indianapolis Coalition for Patient Safety (ICPS)
In 2008, SHO joined the Indianapolis Coalition for Patient Safety, a consortium of Indianapolis hospitals united in a collaborative effort to ensure patient safety (through regional deployment of consistent clinical protocols to address high-risk issues). Each of the SHO hospitals recently selected an ICPS initiative to lead within SHO, with centralized coordination through SHO.

Coalition initiatives are expected to provide consistent care models that are followed within the Central Indiana region, since physicians and nurses travel to various hospital settings. Initiatives focus on care that is vulnerable to adverse events (such as surgery, administration of high-risk IV medications and infection-prone procedures).

The areas of focus for the work groups include:

Measurement Coding Initiative- Hendricks Regional Health
This workgroup has two areas of focus:

  1. Confirm data source & identify ICD-9-CM diagnostic codes used for Infection Mortality graphs provided by IHA
  2. Recommend ICD-9-CM diagnostic codes

Medication Reconciliation- Westview Medical Campus
This workgroup is working towards the launch of a patient education campaign with an emphasis on the medication list, both within and outside of the hospital setting.

Perioperative Safety- Witham Health Services
This workgroup has been asked to provide guidelines for a) the verification & documentation of correct patient identity/ procedure/surgical site, and b) time-outs.

Prevent Catheter-Related Infections- Henry County Hospital
This workgroup is implementing, throughout all Coalition hospital systems, standardized:

  1. Urinary tract catheter insertion & maintenance bundles, based on best practices proven to prevent catheter-associated UTIs
  2. Central venous catheter insertion & maintenance bundles, based on best practices proven to prevent catheter-associated BSIs

Safe Administration of High-Risk IV Medications- Hancock Regional Hospital and Hendricks Regional Health
This workgroup’s efforts are focused on standardizing the concentrations and dosage units for a select group of high-risk IV drugs throughout the Coalition hospital systems.

Severe Sepsiss- Riverview Hospital
This workgroup has two primary tasks:

  1. Implement standard screening criteria and tool (consistent with the Surviving Sepsis Campaign Guidelines) for early identification of sepsis in all Coalition hospital systems.
  2. Implement standard sepsis resuscitation bundle (consistent with the Surviving Sepsis Campaign Guidelines) in all Coalition hospital systems.
