ACO Care Coordinators

The ACO Care Coordinator meeting serves as a forum for discussion, collaboration and dissemination of best practices that help the Suburban Health Organization (SHO) hospitals achieve the Triple Aim of improving patient outcomes, improving experience and lowering cost of care. The meeting also serves as a networking opportunity for the care coordinators to discuss their evolving roles and responsibilities.
